Simvastatin is a lipid-lowering drug from the group of statins (also known as HMG-CoA reductase inhibitors). Its effect is primarily achieved by inhibiting the enzyme HMG-CoA reductase, which is required in the liver for the formation of cholesterol.

The body needs cholesterol, among other things, to transport fats in the blood . The body produces around two thirds of the cholesterol it needs in the liver itself, and one third is obtained from food.

If cholesterol levels are too high, you can reduce your own production, but it often helps to make a fundamental change to your diet. The production of cholesterol in the liver is a complex process.

In effect, it is created from activated acetic acid, a substance that is often found in cells, via many intermediate steps. An important and rate-limiting step in this cholesterol biosynthesis depends on a specific enzyme called "HMG-CoA reductase".

Simvastatin inhibits precisely this step – this reduces the body’s own production and lowers the cholesterol levels in the blood (especially the “bad” LDL cholesterol, while the “good” HDL cholesterol sometimes even increases).

However, statins such as simvastatin are likely to have other positive effects on the body that are independent of the blockade of HMG-CoA reductase. They have a positive effect on vascular function, stabilize deposits (so-called plaques) and suppress the formation of oxidized LDL cholesterol .
